Description

Levitra 5 mg tablet is the reduced strength of vardenafil hydrochloride, a medicine that improves blood flow to the penis during sexual arousal. Most adult men are started on 10 mg, so the 5 mg tablet exists mainly for situations where vardenafil builds up to higher levels in the body than usual. The information below is drawn from current US prescribing information for vardenafil hydrochloride tablets and is meant for patient education. A licensed prescriber decides whether vardenafil is appropriate and at what dose.

Who is started on Levitra 5 mg Pill

The vardenafil label sets 10 mg as the general starting point and allows a range of 5 mg to 20 mg, taken no more than once a day. A 5 mg start is reserved for groups in whom the same tablet produces more drug in the bloodstream or carries a higher risk of low blood pressure. The table summarizes the label-defined situations.

Situation Why exposure or risk is higher Label guidance
Men aged 65 and over In healthy older men, peak level was about 34% higher and total exposure (AUC) about 52% higher than in men aged 18–45. Consider a 5 mg starting dose.
Moderate liver impairment (Child-Pugh B) After a 10 mg dose, peak level rose about 130% and AUC about 160% versus healthy volunteers. Start at 5 mg; do not exceed 10 mg.
Severe liver impairment (Child-Pugh C) Not studied; clearance is expected to be markedly reduced. Do not use.
Stable on an alpha-blocker (for prostate or blood pressure) Both drugs widen blood vessels, adding to the chance of a drop in blood pressure. Start at 5 mg (2.5 mg if also on certain CYP3A4 inhibitors); a time gap between the two doses may be considered.
Taking ketoconazole 200 mg/day, itraconazole 200 mg/day or erythromycin These medicines slow the liver enzyme that clears vardenafil. No more than 5 mg in 24 hours.

Age, liver function and other medicines all change how long a drug stays in circulation, which is why the label builds in these adjustments. A plain-language explanation is available in why some patients need lower starting doses. For vardenafil 5 mg for elderly men, the lower start is a precaution rather than a rule; the prescriber weighs overall health, heart status and the full medicine list. The label also says vardenafil should not be used by people on kidney dialysis, while mild to moderate kidney impairment does not require a change.

How vardenafil works

During sexual arousal, nerves in the penis release nitric oxide, which raises a messenger molecule called cGMP. cGMP relaxes smooth muscle in the erectile tissue so blood can flow in. The enzyme PDE5 breaks cGMP down. Vardenafil blocks PDE5, letting cGMP last longer so the natural erection response is stronger and easier to maintain.

Two consequences follow. First, vardenafil does nothing without sexual stimulation; it is not an aphrodisiac and does not raise libido. Second, the same blood-vessel relaxation elsewhere in the body explains the common side effects (headache, flushing, nasal stuffiness) and the dangerous interaction with nitrates, which also work through the nitric oxide pathway. The liver converts vardenafil mainly through CYP3A4, with smaller roles for CYP3A5 and CYP2C enzymes, into an active metabolite called M1 that contributes roughly 7% of the overall effect.

Taking Levitra 5 mg Tablet

The steps below restate how the prescribing information describes use of Levitra tablets. The pharmacy label and the prescriber’s directions always take priority.

  1. Check the label first. Confirm the strength reads 5 mg and that it is a film-coated tablet, not an orally disintegrating tablet. The two forms are dosed differently.
  2. Plan the timing. The label directs a dose about 60 minutes before sexual activity. In fasting studies, peak blood levels arrived between 30 minutes and 2 hours, with a median of about 1 hour.
  3. Consider the meal. The tablet can be taken with or without food, but a high-fat meal lowered peak levels by roughly 18% to 50% in studies, which can make the onset feel slower or weaker.
  4. Swallow it whole with water. Take one tablet only. Do not add a second tablet if the effect seems mild; vardenafil 5 mg is a maximum of one dose in 24 hours, and some interacting medicines lengthen that interval further.
  5. Allow for the half-life. Vardenafil and its M1 metabolite have a half-life of about 4–5 hours, so the effect window spans several hours. Report how the dose worked, and any side effects, at the next appointment.

Taking an extra dose, mixing vardenafil with another ED tablet, or confusing strengths after a switch in generic supplier are avoidable errors. Common medication mistakes and how to avoid them covers these patterns in more general terms.

What a 5 mg dose can and cannot do

A common question is “is Levitra 5 mg effective?” The honest answer is that it can be, for the right person. In the flexible-dose trials that supported approval, men could be adjusted between 5 mg and 20 mg, and responses varied widely. For men whose bodies clear vardenafil slowly (older men, those with moderate liver impairment, or those on interacting medicines), a 5 mg tablet may produce blood levels similar to what 10 mg gives a younger, healthier man.

Realistic expectations help:

  • It can: support a firmer, longer-lasting erection when sexual stimulation is present, with a lower chance of side effects than higher strengths for many men.
  • It cannot: create desire, cause an erection on its own, treat low testosterone, or address relationship or psychological factors behind ED.
  • It may not: work fully on the first attempt. Anxiety, alcohol, a heavy meal and fatigue all play a part, so several tries under similar conditions give a fairer picture.
  • It should not: be adjusted at home. Moving from 5 mg to 10 mg, or down to 2.5 mg, is a decision for the prescriber, who will consider age, liver function, blood pressure and every other medicine involved.

ED can be an early sign of diabetes, high blood pressure or narrowed arteries, and some medicines also contribute; for example, opioid pain medicines can affect sexual health. The NIDDK overview of erectile dysfunction lists common causes, and the American Urological Association ED guideline describes how clinicians assess and treat the condition. Bringing a list of questions helps; questions to ask at every follow-up visit offers a starting point.

Levitra 5 mg side effects

The table lists adverse reactions reported in at least 2% of men in placebo-controlled trials of 5, 10 and 20 mg vardenafil tablets (2,203 men on vardenafil, 1,199 on placebo), as given in the current generic label. Rates are pooled across strengths, so men on 5 mg alone may see somewhat lower figures.

Adverse reaction Vardenafil Placebo
Headache 15% 4%
Flushing 11% 1%
Rhinitis (stuffy or runny nose) 9% 3%
Dyspepsia (indigestion) 4% 1%
Accidental injury 3% 2%
Sinusitis 3% 1%
Flu-like syndrome 3% 2%
Dizziness 2% 1%
Raised creatine kinase 2% 1%
Nausea 2% 1%
Back pain 2.0% 1.7%

In that trial set, 3.4% of men on vardenafil stopped because of side effects, compared with 1.1% on placebo. Most effects are mild and fade within a few hours as levels fall.

Rare but serious effects

  • Priapism: an erection lasting longer than 4 hours can permanently damage erectile tissue and needs emergency treatment.
  • Sudden vision loss: non-arteritic anterior ischemic optic neuropathy (NAION) has been reported with PDE5 inhibitors. Sudden loss of sight in one or both eyes means stopping the medicine and seeking care immediately.
  • Sudden hearing loss: sometimes with ringing in the ears or dizziness. Stop and get medical help promptly.
  • Heart strain: sexual activity itself raises cardiac workload. Men for whom sex is not advisable because of heart disease should not use vardenafil.
  • Low blood pressure: lightheadedness or fainting, especially with alpha-blockers or other blood-pressure-lowering medicines.

Suspected side effects can be reported to FDA MedWatch, as well as to the prescriber.

The QT question

The QT interval is a measurement on an electrocardiogram (ECG) that reflects how long the heart’s lower chambers take to recharge between beats. When it becomes too long, a dangerous rhythm called torsades de pointes can develop. Among the PDE5 inhibitors, vardenafil is the one whose label carries a specific QT warning: in a dedicated study, single doses produced a modest increase in the corrected QT interval.

For most men this small change has no practical effect. It becomes important when another factor already pushes QT longer. The label advises avoiding vardenafil in men with congenital (inherited) long QT syndrome and in men taking these antiarrhythmic medicines:

  • Class IA antiarrhythmics: quinidine, procainamide
  • Class III antiarrhythmics: amiodarone, sotalol

The label also asks for caution with other medicines known to prolong QT. Some antibiotics, antifungals, antipsychotics and antidepressants fall into this group, and several of them also slow vardenafil clearance, compounding the effect. Low potassium or magnesium, recent heart attack and heart failure add further risk. Clinicians often check lists maintained by CredibleMeds, a reference on QT-prolonging drugs, and may request an ECG first. A lower 5 mg dose does not remove this concern; the label says to avoid the combinations above rather than lower the dose.

CYP3A4 medicines and the 5 mg ceiling

Because CYP3A4 is the main route of vardenafil breakdown, medicines that block this enzyme can raise vardenafil levels several-fold. The label sets maximum doses for named inhibitors, and for some of them even 5 mg is too much. The concept of enzyme inhibitors is explained in how CYP3A4 liver enzymes and their inhibitors change drug levels; the same mechanism applies here.

Interacting medicine Maximum vardenafil dose (label)
Ritonavir; cobicistat 2.5 mg per 72 hours
Indinavir, saquinavir, atazanavir 2.5 mg per 24 hours
Ketoconazole 400 mg/day or itraconazole 400 mg/day 2.5 mg per 24 hours
Clarithromycin 2.5 mg per 24 hours
Ketoconazole 200 mg/day or itraconazole 200 mg/day 5 mg per 24 hours
Erythromycin 5 mg per 24 hours

Only erythromycin and the lower ketoconazole or itraconazole doses leave room for a full 5 mg tablet. Grapefruit juice can also inhibit gut CYP3A4 and is best discussed with the pharmacist. Short antibiotic or antifungal courses are easy to forget, so a prescriber or pharmacist should see the complete medicine list each time something new starts; why doctors check prescription history describes why this review is routine.

Levitra 5 mg vs Staxyn and other forms

Vardenafil has been made in two dosage forms: film-coated tablets (Levitra and its generics) and a 10 mg orally disintegrating tablet (ODT), sold as the brand Staxyn and now available as generic vardenafil ODT. The comparison of Levitra vs Staxyn comes down to the points below.

Feature Vardenafil 5 mg film-coated tablet Vardenafil 10 mg ODT (Staxyn type)
How it is taken Swallowed with water Placed on the tongue to dissolve, without liquid
Strengths Part of a 2.5, 5, 10, 20 mg range 10 mg only; cannot be split or adjusted
Exposure Reference Higher; mean AUC about 44% greater than a 10 mg film-coated tablet in healthy men
Moderate liver impairment Start at 5 mg Do not use
Strong or moderate CYP3A4 inhibitors Allowed at capped doses Do not use
Starting while on an alpha-blocker Start at 5 mg Do not start; switching only on prescriber advice

The ODT label states plainly that it is not interchangeable with film-coated tablets. Men who need a 5 mg starting dose are, by definition, the group for whom the fixed 10 mg ODT is unsuitable. Details are in the vardenafil orally disintegrating tablet label on DailyMed.

Stop and get emergency help if any of these happen after vardenafil

  • An erection that has not gone down after 4 hours, painful or not: treatment within hours protects the tissue.
  • Chest pain, pressure, a racing or irregular heartbeat, or fainting during or after sex: stop at once and call emergency services. Tell responders vardenafil was taken and when, because the label gives no safe waiting period for nitrate-based chest pain medicines after vardenafil, and the emergency team needs that timing to choose treatment.
  • Sudden dimming or loss of sight, or sudden muffled or lost hearing in one or both ears.
  • Any vardenafil taken alongside nitroglycerin, isosorbide, “poppers” or riociguat: blood pressure can fall to dangerous levels.
