Indicated for "management of pain severe enough to require daily, around-the-clock, long-term opioid treatment" in adults and opioid-tolerant pediatric patients ages 11+ already taking at least 20 mg daily oxycodone equivalent. Reserved for patients where "alternative treatment options are ineffective, not tolerated, or would be otherwise inadequate." Not for as-needed use.
